Program Description:

Behavioral health integrated care is increasingly growing in the medical and behavioral health field as we learn about how the mind and body are connected.  This training is designed to provide a brief introduction to integrated care.  Training topics will include implementation models, provider relations, warm-hand offs, and navigating the demands.

Goals:

•             Gain understanding of basic foundational concepts of behavioral health integrated care

Objectives:

•             Participants will be able to define and explain behavioral health integrated care and understand what makes a successful integration experience.

9:30-10:00 am                  Importance of Provider Relationships and Building Medical Provider Relationships

10:00-10:30 am                Referrals and Warm Hand Offs

10:30-11:00 am                Self-Management Strategies for BHI
